Girls Youth Volleyball Registration Form

This volleyball program is an instructional format that will teach the fundamentals through skill development and game play. All game/practice equipment will be provided by The City of Seward. Payment MUST be with registration.

Program runs October 27th – November 17th at Seward Middle School.

3rd grade: 1 p.m.
4th grade – 6th grade: 2 p.m.
Only players will be allowed in the building. Parents must drop off and pick at designated times.

Fee: $30 ($1.50 convenience fee added if payment made online.)

Deadline: October 21st, 2024

I (we) release the City of Seward Parks and Recreation Department and all of its coaches from all claims on account of any injuries which may be sustained by my child while participating in any City-sponsored activity. If medical attention is required for injury or illness, I give my permission for such medical care. I give the Parks/Recreation Department my consent to take and use photographs of my child during these recreational activities.
